You just have to put different name than your's and the address can be the same. If your site's good you'll get approved without any difficulties. If your site is not good, try some guys who offer Adsense accounts for $8-9. They create Adsense account with your name and address, but put their site. Its simple.

As far as Adsense alternative are concerned, there is none near to Adsense, so drop the idea of giving Adsense alternatives a try.
